IIRC...A milled diver extension is breated from a solid piece of metal not stamped from sheet metal :)

Extends the bracelet if the watch is worn over a wet suit (diving) and is made ("milled") from solid steel rather than stamped or forged from sheet metal. A little more expensive to make and typically a sign of a higher end watch.
